Who is eligible right now?
As of March 22, people age 16 and older with certain medical conditions associated with an increased risk of severe illness from COVID-19 are eligible for the vaccine.
Beginning April 5, 2021, everyone age 16 and older will be eligible to receive COVID-19 vaccines.

How to get on the list:
HealthNet of Rock County is taking appointments through two means. You may call to schedule your vaccine or visit www.healthnet-rock.org to register online.
How to make an appointment:
Please contact 608-756-4638 to schedule an appointment. When reaching the phone tree, please press 1 to speak with the Rex Kolste Medical Clinic.
Due to high call volume from current patients and the general public, you may be asked to leave a voice message. HealthNet of Rock County will try to respond to your voicemail within 24 hours or less.
You may visit https://healthnet-rock.org/covid-19-vaccine-information/ to register. Someone will contact by telephone after you register to schedule an appointment.
Due to HIPAA regulations, no appointments from the general public can be scheduled over email or Facebook.
