NOTICE: If you cannot make your appointment, please be sure to cancel in advance. 

Who is eligible right now?

Everyone age 5 and older are welcome. Parental consent is required for minors under the age of 18. Patients under the age of 11 must be accompanied by a parent or guardian.
Booster doses are available for anyone who qualifies. You do not need to show identification or proof of residency. The vaccine is free.
The Pfizer vaccine will be administered at this clinic for ages 5+.

Schedule at or call 608-352-6727. Walk-ins are also welcome.

How to make an appointment:

**Beginning Wednesday, November 17th**

Every Wednesday, 3 p.m. – 7 p.m.

*PLEASE NOTE: The clinic will be closed on November 24th and December 22nd due to the holidays*
